This course walks you through the Sterilization Consent Form and what is needed for the form to be accepted by DHCS. You will see the current version of the form—DHCS 8649—which went into effect on October 15th, 2025. You will also review the previous form, PM 330, and discuss when it will be phased out. Along the way, we’ll cover the key requirements for acceptance, including what informed consent really means and why it matters.
